What is clinical decision support?
Clinical decision support is software that brings the right clinical knowledge to the right patient at the moment you need it. Instead of looking up a guideline after the patient has left, the guidance appears inside the consultation: the questions to ask, the examinations to do, the diagnoses to consider and the treatment the guidelines recommend.
PRIME is clinical decision support built for South African primary care. It was designed by South African doctors around the PHC Standard Treatment Guidelines and Essential Medicines List, and it works in the patient's own language. You remain the clinician: PRIME structures the consultation and checks your thinking, and you make the decisions.
In every consultation, PRIME:
- 1Guides a structured history, in the patient's language
- 2Explains the examinations to do, step by step
- 3Ranks the likely diagnoses, with the reasoning
- 4Screens for danger signs
- 5Drafts the guideline prescription
- 6Follows the patient up after the visit
The bigger picture
Nurses are the backbone of South African healthcare!
More than 265,000 nurses are registered with the South African Nursing Council, nearly five for every doctor. In clinics, pharmacies, workplaces and rural communities, the first clinician most South Africans see, and often the only one, is a nurse.
That is why nurse-led care is growing. Nurse-led clinics are opening across the country, and remote, doctor-supported care is becoming normal for patients. A well-run nurse clinic is not only a service to its community: it is a viable small business.

Why patients come back
Better care is the best marketing a clinic can have!
Patients rarely judge a clinic by its waiting room. They judge it by whether they got better, and by whether anyone cared enough to check. That is where loyalty, and word of mouth, come from.
A consultation on your own:
- A rushed history can lead to the clue being missed, which changes the diagnosis.
- A diagnosis made from memory, with no one to check it.
- Create a script, but you are unsure if it is within your scope, therefore you rather refer patient away.
- After patient leaves, you never know whether they got better, unless you manually follow up.
A consultation with PRIME:
- A structured, in-depth history in the patient's own language, with translations.
- A ranked diagnosis with the reasoning behind it, and danger signs screened.
- A guideline prescription. Outside your scope, a doctor on call signs it off and the patient stays with you.
- Every patient followed up on WhatsApp, so they feel cared for, and anyone not improving is flagged to you.
What it does for your practice
Six reasons nurses choose PRIME
- 01
Better diagnoses
Even in well-resourced health systems, about 1 in 20 adults experiences a diagnostic error in outpatient care each year. A structured history and a ranked differential give every patient a careful, complete assessment.

Better prescriptions
Every script is built from the South African STG/EML, with the dose, duration and guideline reference included, and chronic medication carried through.
- 03
Every patient followed up
PRIME checks in with each patient on WhatsApp after the visit. Patients feel remembered and welcome, and you hear early about anyone who is not improving.
- 04
A doctor on call when you need one
Outside your scope, an HPCSA-registered doctor reviews the prescription and case summary remotely, and edits or signs it off. You keep the patient instead of referring them away.
- 05
Know your numbers
Your dashboard shows patients seen, consultations and follow-ups, so you can run your clinic like the business it is.
- 06
Less paperwork
Prescriptions, referral letters and sick notes are drafted from the consultation, ready to sign, so your time goes to patients.
Doctor on call
You are never on your own with a difficult case.
Most of what walks through your door is within your scope, and PRIME helps you manage it with confidence. When a case is not, you do not have to send the patient away. PRIME sends the prescription and a structured case summary to an HPCSA-registered doctor, who edits or signs it off remotely.
The patient is treated in your clinic, you learn from every review, and you pay only R55 for the cash patient cases a doctor reviews, or nothing at all when the patient is on medical aid.
- HPCSA-registered doctors
- Prescription and case summary, reviewed remotely
- Edited or signed off, back to your clinic

Your questions, answered
Will PRIME replace my clinical judgement?
No. PRIME structures the consultation and checks your thinking against the guidelines, but you examine the patient and make the decisions. Many nurses find it sharpens their own judgement over time.
What happens when a patient needs treatment outside my scope?
PRIME recognises it and sends the prescription with a case summary to an HPCSA-registered doctor, who edits or signs it off remotely. That review costs R55 for cash patients. Medical aid patients cost you nothing extra. The doctor's review is billed to their medical aid as a prescription. Everything within your scope is included in your monthly fee.
How are my patients followed up?
After the visit, PRIME checks in with your patient on WhatsApp and tells you if anyone reports that they are not improving, so you can act early.
Which languages can I consult in?
PRIME works in 21 languages, including all 11 official South African languages, with the English translation shown alongside the patient's words.
Do I need special equipment?
No. PRIME runs in a web browser on a computer or tablet. A waiting-room pre-consultation device is optional.
What if PRIME is not right for me?
You are covered by our 30-day money-back guarantee. If you are not satisfied within your first 30 days, we will refund you.
